SQL cluster service fails over several times a week

Our shared cluster disks are iSCSI mounted on an Equallogic PS storage cluster.  There are no iSCSI errors in the event log corresponding to the failover.  This was happening more frequently but I installed the Equallogic Host Integration Tools and the frequency went from almost daily to every couple of days.  Still it causes some issues with the web application that is connecting to the database because it takes too long to migrate and the web servers time out.

The following error is posted in the failover cluster event log:

Event ID: 1587

Cluster file server resource 'FileServer-(ANGELSQLCLUSTER)(Cluster Disk 2)' failed a health check. This was because some of its shared folders were inaccessible. Verify that the folders are accessible from clients. Additionally, confirm the state of the Server service on this cluster node using Server Manager and look for other events related to the Server service on this cluster node.

Event ID: 1069

Cluster resource 'FileServer-(AngelData)(Cluster Disk 3)' in clustered service or application 'AngelData' failed.

The cluster servers are patched current.

Any help would be appreciated.
